Limit one (1) entry form per person, per visit. No facsimiles allowed. Mass entries and reproductions will be disqualified. No limit on mail entries, but only one per outer envelope. Not responsible for late, incomplete or lost entry forms.

2. HOW TO REGISTER TO WIN. Beginning Monday, March 1, 2010, viewers may register to win one of 100 free entries into the Fifth Third 5K Community Walk, taking place Saturday, May 8 in downtown Grand Rapids. . 3. ELIGIBILITY. Drawing is open to all residents of the Continental United States, eighteen years of age or older. Employees of WOOD TV8, WOTV 4, Lin Television, Fifth Third Bank and their advertising agencies, consultants, franchisees, suppliers, subsidiaries and family members of any of the above are not eligible to enter or win.

4. CONTEST DEADLINES. This contest ends on Wednesday, March 31, 2010.

5. PRIZES. WOOD TV8 and Fifth Third Bank guarantee that 100 free entries into the 5K Community Walk will be awarded.

6. ODDS OF WINNING. 100 winners will be randomly drawn and the odds of winning are based on the number of entries recieved .

7. PRIZE RESTRICTIONS. Prize is non-transferable. No substitute or cash redemption will be awarded. All income and other taxes are the responsibility of the winner. By entering, winners consent to the use of their name, likeness, and/or biographical information for advertising and promotional purposes without additional consideration.

8. DISCLAIMER OF LIABILITY. Winner is responsible for income and other taxes on prize. WOTV 4 and participating entry sites are not liable for lost or stolen entries. Winner will be required to sign and return an Affidavit of Eligibility, Release of Liability and Letter of Indemnification within fourteen (14) days of notification of being a winner. Failure to comply with these conditions may result in forfeiture of prize and alternative prizewinner being selected from the remaining eligible entries.

9. MISCELLANEOUS. Decisions made by the WOTV 4 judges concerning any and all matters with respect to this promotion shall be final.
